What do I need to bring for my first appointment?
For your first appointment at The Smile Studios, we do not require you to bring anything. Once at the office, we will ask you to accomplish a Confidential Medical & Dental History form, which you can also opt to do at home beforehand if you want to save time. The form can be downloaded from our website. We will be reviewing your history with you before we begin with our first comprehensive dental and oral check-up. It is important that you disclose everything that’s asked in the form truthfully since your answers will be helping us in the development of your dental and oral health care and treatment plan.
You can request for your first appointment online or by calling us at 020-8332-6833 for our Richmond office, 020-8882-8070 for our Park Parade in Palmer’s Green office, or 020-8577-2206 for our Heston office. If you are more comfortable writing down your request or inquiry, you can fax it to us at at 020 8948 6287 for Richmond, 020 8886 5507 for Park Parade or 020 8577 9181 for Heston, or you can email us here. Get in touch with us now and we will give you directions on how you can reach whichever of our office is nearest your place. All our dental offices are easily accessible to both public and private transportation. At Smile Studios, we are already open at 8:30 AM, Mondays through Saturdays. At our Richmond office in Surrey, we are even open on Sundays, from 10 AM to 3 PM.
